The following information is intended to help you obtain any insurance reimbursement to which you are entitled.
Bring the insurance information to your appointment: including name of subscriber, Date of Birth, employer, insurance company name, policy/group #, and subscriber's ID#.
Our office is set up to send claims and predeterminations electronically to some insurance carriers. If we are unable to submit electronically, we will provide you with a manual claim form which you can mail in. All correspondence and cheques from your insurance carrier will be sent to the subscriber.
Most insurance plans do not cover 100% of the specialist fees. You can usually expect to be reimbursed 50 - 80%. We will provide you with a dental predetermination to submit to your insurance carrier. They will let you know how much they will cover.
We do not accept assignment of payment from your insurance carrier. The insurance cheque will be sent to the subscriber. We request payment in full at each appointment. We accept cash, debit, VISA, MC.
